Key Takeaways
Most hair transplants ask you to wait a year to see what you have paid for. Long hair FUE is different: the grafts are moved with their hair still long, so when the operation ends you can look in the mirror and see roughly where your new hairline will be. For some patients that preview, and the discretion that comes with it, is worth a lot.
It also comes with limits that are rarely explained. On this page I explain what long hair FUE is, how it differs from an unshaven or DHI procedure, what really happens to the preview after surgery, where the technique works best, what the research shows and what to expect from recovery and cost. If your main goal is simply to avoid shaving, read my unshaven hair transplant page first. Extraction itself can be done by hand; see my manual FUE page.
What Is Long Hair FUE?
Long hair FUE is a form of FUE (follicular unit excision) in which the hair on each graft is not shaved or trimmed to stubble. The donor area is left at its normal length, each graft is freed with a punch designed to work around long hair, and the graft is implanted with its hair still attached, usually shortened to a few centimetres so it can be handled.
Long Hair FUE vs Unshaven FUE, DHI and Shaven FUE
These techniques are often mixed up in advertising. This is how they differ:
Long hair FUE is the most discreet option and the only one with a preview, but also the slowest. For a large area, the same grafts can usually be moved faster and in greater numbers with Sapphire FUE or DHI. You can compare all methods on my hair transplant techniques page.
The Preview: What You See on Day One, and What Happens Next
The preview is the reason most patients ask for long hair FUE, so it is important to understand what it is and what it is not.
So the preview is temporary. For a few weeks after the long hairs shed, the area can look much like it did before surgery. Patients who know this in advance find it easy; patients who do not can find it upsetting.
How the Preview Helps Me During Surgery
The preview is not only for the patient. It is also one of the most useful things about the technique for me as a surgeon.
- Seeing the hairline as I build it: with long hairs in place, I can see how the front edge falls and adjust its shape and irregularity while the operation is still going on.
- Matching direction and curl: long hairs show which way each graft wants to lie, which matters most at the temples, in eyebrows and in curly hair.
- Placing density where it shows: I can see where the existing hair already gives coverage and put grafts where they make the most visible difference.
- Choosing donor hairs: in the donor area, long hair shows the curl and calibre of each hair before it is taken, which helps match grafts to the recipient area.
A well-designed hairline is the most important part of any hair transplant, whatever the technique. I explain how I design it in this video.

Where Long Hair FUE Works Best
When I recommend something else
- Advanced hair loss (Norwood 4 and above) or a large area to cover: the session would be too long. Sapphire FUE or a staged plan is usually better.
- Very short hair at the back of the head, which cannot hide the donor area.
- A weak donor area, where full visibility of the scalp helps protect the remaining grafts.
- Patients who expect the day-one look to stay. The long hairs will shed.
Curly and Afro-textured hair can be done with long hair FUE, but extraction is slower and the preview can take a few more days to settle. See Afro hair transplant.
What the Research Shows About Long Hair FUE
Long hair FUE is newer than shaved FUE, but there is now useful published experience.
Before Your Long Hair FUE Operation
- Let your hair grow at the back, sides and front in the weeks before surgery. I will tell you how long it should be once I have seen your photos.
- Send clear photos of the front, top, sides and back, plus one with the hair at the back lifted, so I can assess the donor area.
- Do not colour or chemically treat your hair in the weeks before surgery, unless I advise otherwise.
- Wash your hair on the morning of surgery with shampoo only, without conditioner, oils or styling products.
- Plan time for a longer day: long hair FUE takes more time than a shaved procedure.
Caring for Long Transplanted Hair
The main difference after long hair FUE is that the transplanted hairs are long and can catch on combs, pillows and clothes while the grafts are still settling.
In this video my colleagues and I answer common questions before surgery, including what long hair FUE and an unshaven hair transplant are.

How Much Does Long Hair FUE Cost?
Long hair FUE usually costs more than a shaved procedure at the same clinic, because it takes much longer per graft. These are typical 2026 ranges for a hair transplant before any long hair surcharge.
When comparing quotes, check that ‘long hair FUE’ really means the transplanted hairs stay long. Some clinics use the name for an unshaven procedure in which the grafts are trimmed short.

Frequently Asked Questions About Long Hair FUE
What is long hair FUE?
Long hair FUE is an FUE hair transplant in which the donor area is not shaved and the grafts are implanted with their hair still long, so you can see a preview of the result on the day of surgery.
Is long hair FUE the same as an unshaven hair transplant?
No. Both avoid a visible shave, but in an unshaven procedure the grafts are usually trimmed short before placement. In long hair FUE they stay long, which creates the preview.
Will the long transplanted hairs stay?
No. Most of them shed within about two to eight weeks. The follicles stay under the skin and start growing new hair from around month three, with the final result at 12 to 18 months.
Is the final result the same as the preview?
Not exactly. The preview shows the design and direction. The final result can look slightly denser or slightly different in places, because the new hair grows and thickens over a year.
How many grafts can be done with long hair FUE?
Fewer than with a shaved procedure, because the work is much slower. In published series, sessions averaged around 1,200 grafts for female hairlines and 1,500 for male hairlines. It is best for small, visible areas.
Who is long hair FUE best for?
People with early hair loss at the hairline, women who want to lower or reshape their hairline, eyebrow and beard cases, scar repair, and anyone who cannot be seen with a shaved head.
Is long hair FUE good for eyebrows?
Yes. The long hairs show their natural curl, which helps match the direction of eyebrow hair, and studies of eyebrow transplants with this technique report high satisfaction.
Is long hair FUE more expensive?
At most clinics, yes, because it takes longer. My Diamond package price stays the same for long hair FUE.
How long does it take to recover?
Most patients return to desk work within a few days because the treated area is hidden by hair. The grafts are anchored after about 10 to 14 days.
